Clear slips are no longer issued. You register online and the roster is shared with the coaches.
Sports Qualifying Physical - This physical is usually completed in the 7th and 10th grades. Students are not allowed to compete without the forms on file in our nurses office. Your doctor's office will also have copies of this form. The physical is valid for 3 years from the date of the physical. The physical must be brought to the activities office so that it can be authenticated and registered in our system.
Concussions Impact Form - In partnership with the Mankato Clinic Foundation, St. Peter Schools initiate a baseline concussion test for those in grades 7, 9, 11 and any other first time athlete in grades 8, 10 and 12. This test is performed during school hours on computers and is kept on file in case an athlete suffers a concussion. More information is available here.
Transfer Students - If you are new to our school, 9th grade or older and wish to participate on an athletic team, please see Mr. Alger to determine your eligibility status.

Onc you've completed the appropriate forms, you can bring them, along with the appropriate fees to the Activities Office.
Clear slips are NOT issued when you have equipment or uniforms out from preceding seasons. All uniforms must be checked in with the coach at the end of each season.
Facility Use Form - If you wish to use space at St. Peter High School.
Check Request - This form should be completed by advisors or coaches to reimburse for team expenses or club expenses. This includes any boosters expenses that need to be reimbursed by the school.
Bus Request - This form should be completed by advisors, coaches or teachers requesting field trips, shuttle buses, or other transportation needs. You should make sure that your trip shows up on our co-curricular schedule within 48 hours of a request. The organizer will also receive a 'notifyme' email to show that is has been placed on the schedule.
Key Request - If you use the gym space and or equipment to coach a team and need access. Please complete a key request form. (Coming soon 4/2015)
Varsity Level Officials
Occasionally we have officials work an event at our school at the varsity level that have not officiated at our school in the past. In order to get paid, varsity officials must complete a W9 and submit to the activities office.
If you are working an event (line judges, ticket sellers, ticket takers, officiating Middle School games, announcer, etc), effective 1/1/2016 you will be paid through payroll. Payroll requests that the following forms to be completed and submitted to the District Office before you will be paid:
Payroll - General form to collect basic information for your personnel file.
W-4 – this form determines the level of withholding from each check. You only need to submit page 1 of the form. The 2nd page is a worksheet that assists you with determining adjustments/deductions.
Direct Deposit Form – You will be paid twice a month. Your first check will be mailed. If you complete this form, future checks will be directly deposited to a checking or savings account.
I-9 Form - Print pages 7-9. Complete page 7 of this form and provide copies of the appropriate identification from the lists on page 9. (Ex: Passport copy, or copy of driver's license and social security card) Review page 9 for other acceptable forms of identification, but these are the most common submissions.
Once you’ve completed all forms you can submit them to the District Office.
