Grade

( 5 ) Consistently Exceeds Expectations

Student always wears appropriate attire correctly (ie-shoes tied).

Student demonstrates all skills at grade level.

Student consistently demonstrates a high level of enthusiasm, self-control,

sportsmanship, and follows all safety rules while participating in all activities.

Student consistently worsk cooperatively with classmates.

Student helps with cleaning up equipment without being asked.

Student wears appropriate attire.

Student fully participates in all physical education classes.

Student demonstrates a high level of enthusiasm, self-control,

sportsmanship, and follows all safety rules while participating in all activities.

Student consistently works cooperatively with classmates.

Student frequently wears the appropriate Physical Education attire.

Student willingly participates in class with minimal encouragement.

Student often demonstrates enthusiasm, self-control, sportsmanship,

and follows all safety rules while participating in activities.

Student regularly works cooperatively with teammates.

Student occasionally wears appropriate physical Education attire.

Student participates with frequent encouragement.

Student occasionally demonstrates enthusiasm, self-control, sportsmanship

and shows a limited willingness to follow safety rules.

Student displays limited initiative to function as a team player.

( 1 ) Improvement Needed

Student rarely wears appropriate Physical Education attire.

Student displays a reluctance to participate in physical education activities.

Student has little or no self-control, poor sportsmanship, and shows an

unwillingness to follow safety rules.

Student rarely displays initiative to function as a team player.
